9c6da3b7f1 scsi: message: mptlan: Fix use after free bug in mptlan_remove() due to race condition

mptlan_probe() calls mpt_register_lan_device() which initializes the
&priv->post_buckets_task workqueue. A call to
mpt_lan_wake_post_buckets_task() will subsequently start the work.

During driver unload in mptlan_remove() the following race may occur:

22cf11e162 FROMGIT: scsi: core: Change the return type of .eh_timed_out()
Commit 6600593cbd ("block: rename BLK_EH_NOT_HANDLED to BLK_EH_DONE")
made it impossible for .eh_timed_out() implementations to call
scsi_done() without causing a crash.

Restore support for SCSI timeout handlers to call scsi_done() as follows:

 * Change all .eh_timed_out() handlers as follows:

   - Change the return type into enum scsi_timeout_action.
   - Change BLK_EH_RESET_TIMER into SCSI_EH_RESET_TIMER.
   - Change BLK_EH_DONE into SCSI_EH_NOT_HANDLED.

 * In scsi_timeout(), convert the SCSI_EH_* values into BLK_EH_* values.

Just a few lines below this kzalloc() we have a mutex_lock() which can
sleep.

Moreover, the only way to call this function is when a delayed work is
schedule. And delayed work can sleep:

  INIT_DELAYED_WORK(&fw_event->work, mptsas_firmware_event_work);
    --> mptsas_firmware_event_work()
      --> mptsas_send_link_status_event()
        --> mptsas_expander_add()

So there is really no good reason to use GFP_ATOMIC here. Change it to
GFP_KERNEL to give more opportunities to the kernel.

Some reasons why this API should be removed have been given by Julia
Lawall in [2].

When memory is allocated in kbuf_alloc_2_sgl() GFP_KERNEL can be used
because this function already uses the GFP_USER flag for some memory
allocation and not spin_lock is taken in the between.

When memory is allocated in mptctl_do_mpt_command() GFP_KERNEL can be used
because this function already uses copy_from_user() and this function can
sleep.

When memory is allocated in mptctl_hp_hostinfo() GFP_KERNEL can be used
because this function already uses mpt_config() and this function has an
explicit might_sleep().

When memory is allocated in mptctl_hp_targetinfo() GFP_KERNEL can be used
because this function already uses mpt_config() and this function has an
explicit might_sleep().

This series consists of the usual driver updates (ufs, smartpqi, lpfc,
 target, megaraid_sas, hisi_sas, qla2xxx) and minor updates and bug
 fixes.  Notable core changes are the removal of scsi->tag which caused
 some churn in obsolete drivers and a sweep through all drivers to call
 scsi_done() directly instead of scsi->done() which removes a pointer
 indirection from the hot path and a move to register core sysfs files
 earlier, which means they're available to KOBJ_ADD processing, which
 necessitates switching all drivers to using attribute groups.

The wrappers in include/linux/pci-dma-compat.h should go away.

The patch has been generated with the coccinelle script below and has been
hand modified to replace GFP_ with a correct flag.  It has been compile
tested.

When memory is allocated in 'mptfc_GetFcDevPage0()' GFP_KERNEL can be used
because it is already used in this function and no lock is acquired in the
between.

When memory is allocated in 'mptfc_GetFcPortPage0()' and
'mptfc_GetFcPortPage1()' GFP_KERNEL can be used because they already call
'mpt_config()' which has an explicit 'might_sleep()'.

817a7c9967 scsi: message: fusion: Remove in_interrupt() usage in mptsas_cleanup_fw_event_q()
mptsas_cleanup_fw_event_q() uses in_interrupt() to determine if it is safe
to cancel a worker item.

Aside of that in_interrupt() is deprecated as it does not provide what the
name suggests. It covers more than hard/soft interrupt servicing context
and is semantically ill defined.

Looking closer there are a few problems with the current construct:

 - It could be invoked from an interrupt handler / non-blocking context
   because cancel_delayed_work() has no such restriction. Also,
   mptsas_free_fw_event() has no such restriction.

 - The list is accessed unlocked. It may dequeue a valid work-item but at
   the time of invoking cancel_delayed_work() the memory may be released or
   reused because the worker has already run.

mptsas_cleanup_fw_event_q() is invoked via mptsas_shutdown() which is
always invoked from preemtible context on device shutdown.  It is also
invoked via mptsas_ioc_reset(, MPT_IOC_POST_RESET) which is a
MptResetHandlers callback. The only caller here are mpt_SoftResetHandler(),
mpt_HardResetHandler() and mpt_Soft_Hard_ResetHandler(). All these
functions have a `sleepFlag' argument and each caller uses caller uses
`CAN_SLEEP' here and according to current documentation: | @sleepFlag:
Indicates if sleep or schedule must be called

So it is safe to sleep.

Add mptsas_hotplug_event::users member. Initialize it to one by default so
mptsas_free_fw_event() will free the memory.  mptsas_cleanup_fw_event_q()
will increment its value for items it dequeues and then it may keep a
pointer after dropping the lock.  Invoke cancel_delayed_work_sync() to
cancel the work item and wait if the worker is currently busy. Free the
memory afterwards since it owns the last reference to it.

>From reading the mpt_config() code and the history this is clearly a debug
mechanism and should probably be replaced by might_sleep() or completely
removed because such checks are already in the subsequent functions.

Remove the in_interrupt() references and replace the usage in mpt_config()
with might_sleep().

311950f8b8 scsi: mptfusion: Don't use GFP_ATOMIC for larger DMA allocations
The mpt fusion driver still uses the legacy PCI DMA API which hardcodes
atomic allocations.  This caused the driver to fail to load on some powerpc
VMs with incoherent DMA and small memory sizes.  Switch to use the modern
DMA API and sleeping allocations for large allocations instead.  This is
not a full cleanup of the PCI DMA API usage yet, but just enough to fix the
regression caused by reducing the default atomic pool size.

afe89f115e scsi: mptscsih: Fix read sense data size
The sense data buffer in sense_buf_pool is allocated with size of
MPT_SENSE_BUFFER_ALLOC(64) (multiplied by req_depth) while SNS_LEN(sc)(96)
is used when reading the data.  That may lead to a read from unallocated
area, sometimes from another (unallocated) page.  To fix this, limit the
read size to MPT_SENSE_BUFFER_ALLOC.

a7f7f6248d treewide: replace '---help---' in Kconfig files with 'help'
Since commit 84af7a6194 ("checkpatch: kconfig: prefer 'help' over
'---help---'"), the number of '---help---' has been gradually
decreasing, but there are still more than 2400 instances.

This commit finishes the conversion. While I touched the lines,
I also fixed the indentation.

There are a variety of indentation styles found.

  a) 4 spaces + '---help---'
  b) 7 spaces + '---help---'
  c) 8 spaces + '---help---'
  d) 1 space + 1 tab + '---help---'
  e) 1 tab + '---help---'    (correct indentation)
  f) 1 tab + 1 space + '---help---'
  g) 1 tab + 2 spaces + '---help---'

In order to convert all of them to 1 tab + 'help', I ran the
following commend:
